Present the applicant’s Report Card (original/photocopy) to the Guidance Office for assessment and entrance exam.
ADMISSION REQUIREMENTS
- Fully accomplished *Application Form
- Photocopy of the Following (present original copies for verification):
– Report Card PSA
– Birth Certificate
– Baptismal Certificate - Good Moral Certificate (Original)
- Medical Certificate (Original)
- Two (2) pieces recent 1×1 picture
- Early Childhood Care and Development (ECCD) Checklist
